• Dr. Josh Baker, West Fork Family Medicine in Rockwell, will conduct school   physicals for students at West Fork Middle and High Schools.  Physicals at the high school will be Friday, May 28 in the north gym starting at 8:30 a.m. until done.   Physicals at the middle school will be Wednesday, June 2 from noon until done.  There will be a $30 charge.   Checks are to be made out to the West Fork Booster Club as all proceeds will be donated to the booster club.  A consent form will be needed for students under 18 and is available for download on the website or in the middle or high school office.

 

• Franklin County Public Health and Iowa Department of Public Health are joining together to update children with the Tdap immunization.  This vaccination is a booster for tetanus, diphtheria and pertussis that is routinely given at about 13 to 14 years old.  We plan to set up clinics next fall during the school day to provide this immunization free of charge.   The clinic will be open for children in grades 7 through 12 (their grade next year).    Forms will be sent home with students that need to be completed and returned to the office by MAY 21^ST.  This form doesn’t obligate you but rather assists us in ordering the amount of vaccine needed.

****SHUTTLE BUS SCHEDULE FOR SPRING ACTIVITIES:   Monday, Tuesday, Thursday, and Friday – shuttle will leave Rockwell at 5:10 and proceed to Swaledale, Thornton and Sheffield and returns to Rockwell at 6:00.     WEDNESDAY SHUTTLE LEAVES AT 4:50 and proceeds to Sheffield, Thornton , Swaledale and Rockwell.
